Academic requirements

Completed application form, a motivation statement, one recommendation letter from a school official, school transcripts of the last two to three years, CV and educational history form, results of a standardised test (SAT or ACT), results of a language proficiency test (TOEFL, IELTS, CAE, Duolingo, etc.).
